NOTICE TO BIDDERS MUNICIPALITY OF MT. LEBANON ALLEGHENY COUNTY, PENNSYLVANIA Sealed Proposals will be received by the Mt. Lebanon Commission at the Municipal Building, 710 Washington Road, Pittsburgh, PA 15228 until 11:00 a.m. prevailing time on Friday December 20, 2013 for the following: SANITARY SEWER MANHOLE REHABILITATION PROGRAM Major bid items are: The rehabilitation of sanitary sewer manholes located throughout the Municipality. Proposals shall be delivered in a sealed envelope that is clearly marked on the outside with the appropriate wording "SANITARY SEWER MANHOLE REHABILITATION PROGRAM" to the desk of the Director of Public Works, Thomas Kelley, Municipal Building, 710 Washington Road, Pittsburgh, PA 15228 before 11:00 a.m. prevailing time, December 20, 2013 delivered to the 1st Floor Service Center, and the same will be publicly opened and read at that time at the Municipal Building Commission Chambers located at 710 Washington Road, Pittsburgh, PA 15228. Prevailing wages will not be required to be paid on the manhole rehabilitation project. SANITARY SEWER POINT REPAIR CONTRACT Sanitary sewer repairs including any one or a combination of the following items: sanitary sewer point repair, removal and replacement of standard sanitary manhole, installation of new standard sanitary manhole, removal of lamphole and installation of standard sanitary manhole, service wye reinstatement, lateral replacement, concrete anchors, concrete encasement, installation of lateral inspection tee, full-depth bituminous street restoration, brick street restoration with concrete base, concrete street restoration, concrete roll curb restoration, concrete curb and gutter restoration, concrete straight curb restoration, bituminous wedge curb restoration, concrete sidewalk restoration, concrete driveway restoration, bituminous driveway restoration, aggregate driveway restoration, brick/paver-stone driveway restoration, fence restoration, and topsoil and seeding. LEBANON STORM & SANITARY SEWER TELEVISING & CLEANING CONTRACT This project includes storm & sanitary sewer televising investigations including the cleaning (light and heavy), plugging/bypass pumping, and close circuit televising of sewers within Mt. Lebanon of various sizes. Proposals shall be delivered in a sealed envelope that is clearly marked on the outside with the appropriate wording "Mt. Lebanon Storm & Sanitary Sewer Televising & Cleaning Contract" to the desk of the Director of Public Works, Thomas Kelley, Municipal Building, 710 Washington Road, Pittsburgh, PA 15228 before 11:00 a.m. prevailing time, December 20, 2013 delivered to the 1st Floor Service Center, and the same will be publicly opened and read at that time at the Municipal Building Commission Chambers located at 710 Washington Road, Pittsburgh, PA 15228. Prevailing wage rates will not be required on the storm & sanitary sewer televising & cleaning contract. Copies of Drawings, Specifications, Instruc- tions to Bidders, General Conditions, Forms of Proposals and Agreements are on file and open to public inspection at the office of The Gateway Engineers, Inc., 400 Holiday Drive, Suite 300, Pittsburgh, PA 15220 where sets of said documents may be obtained on December 5, 2013, upon payment of $50.00 for each of the contracts. No refund or exchange will be made for the return of any documents. An additional $10.00 will be required for each set of plans if they are to be mailed. Performance and Labor and Material Payment Bonds, along with Public Liability and Property Damage Certificates of Insurance in the amounts specified, as well as Certificates of Workman's Comp- ensation must be filed with the executed Agreement. Proposals to receive consideration must be accompanied by a Certified Check or Bidder's Bond from a Surety Company authorized to do business in Pennsylvania, made to the order of Municipality of Mt. Lebanon in an amount equal to ten percent (10%) of the total amount of the Proposal as a guarantee that, if the Proposal is accepted, the successful Bidder will enter into an Agreement within 15 days after Notice of the Award of the Contract. The Commission of Mt. Lebanon, Pennsylvania reserves the right to waive any formalities or to reject any or all bids. If more time is necessary for the ascertainment of the lowest responsible bidder, the Commission of Mt. Lebanon reserves the right to award the contract at a later date, but not exceeding sixty (60) days from the date bids were submitted, and such date will be publicly announced immediately after the bids are opened.
